The role of a Support Coordinator in the National Disability Insurance Scheme (NDIS) is pivotal in assisting individuals to navigate and access community services, supports, and resources.
- Main Responsibilities:
- Support Coordination and Case Management:
- Assess and understand participant needs, preferences, and goals by working closely with NDIS participants and their families or carers.
- Develop and implement tailored support plans in collaboration with participants, ensuring their NDIS goals are met in a timely and effective manner.
- Provide ongoing coordination and monitoring of services and supports to guarantee satisfaction with services and timely goal attainment.
- Maintain comprehensive knowledge of the NDIS funding process and ensure that participant budgets are managed appropriately.
- Ensure timely service delivery and resolve any issues or barriers to service delivery promptly.
- Liaison and Stakeholder Management:
- Foster positive working relationships with NDIS participants, service providers, and other key stakeholders through regular communication and collaboration.
- Promote advocacy on behalf of participants to secure optimal services available, including identifying suitable providers and recommending them to participants.
- Collaborate with healthcare professionals, therapists, and other service agencies to deliver holistic support to participants.
- Guarantee that all provided services align with NDIS plan goals, standards, and guidelines.
- Progress Monitoring and Reporting:
- Track and monitor progress of NDIS plans, conducting timely reviews and amendments as necessary.
- Maintain accurate and up-to-date case notes, reports, and documentation in compliance with organizational policies and NDIS requirements.
- Deliver regular updates and progress reports to participants, families, and the NDIS where appropriate.
- Participant Education and Empowerment:
- Educate participants and their families on how to effectively navigate the NDIS system and make informed choices regarding their supports and services.
- Empower participants to become confident and independent in managing their NDIS plans and connecting with the services they need.
- Compliance and Quality Assurance:
- Ensure all work adheres to the NDIS Code of Conduct, relevant legislation, and organizational policies.
- Contribute to continuous improvement efforts by providing feedback and participating in training and development opportunities.
