Job Description:

As a key member of our clinical team, you will be responsible for coordinating and delivering restorative and rehabilitative care services that promote independence, functional recovery, and optimal health outcomes for clients. You will work closely with stakeholders to assess and meet the unique needs of individuals in need.

Responsibilities:

  • Coordinate community engagement initiatives
  • Assess client needs and develop personalized care plans
  • Provide ongoing support, advice, and guidance to clients and caregivers
  • Ensure quality care processes and risk management practices are implemented

Requirements:

  • Current registration as a Registered Nurse or Allied Health professional
  • Specialist certifications or qualifications in relevant disciplines
  • Extensive experience in clinical coordination, case management, or restorative care within aged care, disability, or community services
  • A strong understanding of reablement, functional rehabilitation, and person-centred care models
  • Proficiency in risk management, compliance, and quality assurance
  • Excellent communication, problem-solving, and decision-making skills
  • Competence in clinical documentation and care management systems
  • Exceptional customer service, interpersonal, and liaison skills
  • Experience managing a high workload and providing high-quality advice and support for diverse client needs
