About the Role:

Care Partners play a pivotal role in delivering person-centered care to older Australians, ensuring they receive timely and high-quality support.

Key Responsibilities:
  • Assess Client Needs: Conduct thorough assessments to identify physical, emotional, and social needs, creating tailored care plans that meet client goals and objectives.
  • Care Planning: Develop, review, and update comprehensive care plans aligned with client aspirations and Support at Home funding.
  • Coordinate Services: Organise in-home supports, collaborating with external providers to ensure seamless delivery of quality care.
  • Advocate and Educate: Empower clients to understand their funding, choices, and rights, advocating for access to appropriate services.
  • Budget Oversight: Monitor individual budgets, guiding clients on effective use of funds to achieve optimal outcomes.
  • Relationship Building: Foster strong connections with clients, carers, and providers, facilitating smooth transitions and ongoing care.
  • Compliance and Safety: Maintain accurate documentation, adhering to aged care standards, and proactively managing risks to ensure a safe environment.

Our Ideal Candidate:

  • Possesses excellent communication skills, is highly organised, and demonstrates advanced problem-solving abilities.
  • Holds a Certificate III in Individual Support or equivalent qualification.
  • Maintains a current Medication competency.
  • Has a First Aid and CPR qualification, or is willing to obtain one.
  • Hold a current Police Check, or is willing to obtain one.
  • Has a National Personal Insolvency Check, or is willing to obtain one.
  • Possesses a current driver's license and a reliable, registered vehicle suitable for work purposes.
