Job Description:

The role of Care Partner involves providing hands-on leadership to a team of Home Care Workers, ensuring older Australians receive high-quality, person-centered care. As a vital link between clients and the Support at Home program, you will conduct initial and ongoing assessments, develop personalized care plans, coordinate services, and advocate for access to appropriate resources.

Key Responsibilities:
  • Conduct thorough client assessments to identify physical, emotional, and social needs
  • Develop, review, and update person-centered care plans aligned with client goals
  • Coordinate in-home supports and liaison with external providers to ensure timely and quality care
  • Support clients in understanding their funding, choices, and rights, and advocate for access to appropriate services
