The Role
As a Dementia Consultant, you will take a person-centred care approach to provide assessments for individuals living with dementia who are showing responsive behaviour. You will work collaboratively to understand their dementia journey, identify triggers and unmet needs, and develop recommendations for improved care and support.
Key Responsibilities
- Provide face-to-face assessments for individuals living with dementia.
- Develop and implement care plans that meet the individual's needs.
- Work collaboratively with healthcare professionals, family members, and carers to ensure seamless care.
- Stay up-to-date with the latest evidence-based dementia care practices and share knowledge with the team.
Requirements
- Strong aged care experience working with individuals living with dementia.
- Experience in senior nurse, clinical care manager, occupational therapist, physiotherapist, diversional therapist, or other dementia-specialised roles.
- Knowledge of different types of dementia and experience in managing BPSD.
- Ability to identify triggers and unmet needs.
