The Role

As a Dementia Consultant, you will take a person-centred care approach to provide assessments for individuals living with dementia who are showing responsive behaviour. You will work collaboratively with individuals, their families, and carers to understand their dementia journey and identify triggers and unmet needs.

Key Responsibilities

  • Provide face-to-face assessments for individuals living with dementia.
  • Develop and implement care plans that meet the individual's needs.
  • Work collaboratively with healthcare professionals and carers to ensure seamless care.
  • Stay up-to-date with the latest evidence-based dementia care practices.

Requirements

  • Strong aged care experience working with individuals living with dementia.
  • Experience in successfully managing Behavioural and Psychological Symptoms of Dementia (BPSD).
  • Knowledge of different types of dementia and ability to identify triggers and unmet needs.
  • Strong communication and interpersonal skills.
