About the Role

We are seeking a highly skilled and experienced Dementia Consultant to join our team at HammondCare. As a Dementia Consultant, you will play a critical role in delivering Australian Government funded dementia behaviour support services.

As a Behaviour Support Specialist, you will work closely with families, caregivers, and clinical staff to understand the impact of dementia on individuals and equip them with the skills and knowledge to prevent and respond to behaviour changes.

Key Responsibilities:

  • Behaviour Support Planning: Develop and implement behaviour support plans that are tailored to individual needs and circumstances.
  • Assessment and Intervention: Conduct comprehensive assessments to identify the underlying causes of behaviour changes and develop effective intervention strategies.
  • Training and Capacity Building: Provide training and capacity building to caregivers and clinical staff on best practice behaviour support strategies.
  • Report Writing: Develop high-quality reports that provide recommendations for behaviour support and inform care planning.
  • Collaboration and Partnership: Work closely with the broader HammondCare team to build capability across the home and aged care sectors.

Requirements

  • Expertise in supporting people living with dementia.
  • Relevant qualifications in Allied Health, Nursing, Aged Care or Mental Health.
  • Experience in Australian residential aged care and/or community aged care.
  • High level of negotiation, communication and relationship building skills.
  • Excellent problem solving and decision-making skills.
  • Ability to prioritise multiple tasks and meet deadlines.
  • Excellent written and verbal communication skills.
  • Excellent stakeholder management.
  • A current Australian drivers licence and willingness to travel.
