Empower Independence in Home Care Roles

We are seeking dedicated and compassionate individuals to join our home care team as Home Support Team Members (HSTMs).

As an HSTM, you will play a vital role in enabling older adults to live independently in their homes while maintaining their dignity and lifestyle.

Working one-on-one with clients, HSTMs provide personalized care and support that makes independence possible. Each day brings opportunities to make a meaningful difference - whether preparing meals, assisting with personal care, helping with household tasks, or simply being a friendly companion.

HSTMs also coordinate medical support, monitor physical health, cognitive function, and social engagement, ensuring any concerns are addressed promptly through proper channels.

By fostering genuine, respectful relationships, HSTMs help clients maintain control over their daily lives while accessing the support they need to thrive at home.

**Key Responsibilities:**

  • Deliver high-quality care and support to clients in their homes
  • Assist with personal care, meal preparation, and household tasks
  • Coordinate medical support and transportation for appointments
  • Monitor client well-being and report any concerns to management

**Requirements:**

  • Valid driver's licence
  • Reliable vehicle
  • National Police Check
  • Certificate III in Individual Support / Aged Care or equivalent
  • First Aid, CPR and Manual Handling
