About this opportunity:

  • This role is for a Community Care Coordinator specialising in Home Care Packages, who will help seniors access the support they need to maintain their independence and lifestyle choices.

Key responsibilities include:

  • To enhance the quality of life for clients by promoting their well-being through tailored home care services
  • To build strong relationships and networks within the community

Benefits:

  • Lots of autonomy in the role
  • Monday - Friday role with travel involved (can be based in different offices around Adelaide)

Requirements:

  • Significant experience within the Aged Care industry in a similar role
  • Understanding of the Aged Care Sector as well as Home Care Package and other programme guidelines and service requirements
