Disability Support Role

This position involves working directly with individuals living with a disability and/or mental health condition to help them increase their independence, maintain their wellness and ultimately improve the quality of their life.

Key Responsibilities

  • Provide case management services to individuals living with a disability and/or mental illness in accordance with service agreements and plans.
  • Utilize evidence-based practices in the development and implementation of individual support plans.
  • Work one-on-one with clients, providing medication assistance and basic personal care as required.
  • Complete, maintain and store documentation in a confidential manner.

Required Skills and Qualifications

  • Good availability across multiple days, evenings and overnight shifts per week.
  • A passion for learning and development.
  • Effective and professional interpersonal and communication skills.
  • Accurate and timely decision-making skills.
  • Ability to work in a fast-paced environment.
