Lutheran Services is seeking a dedicated professional to oversee and manage home care services, ensuring clients receive tailored support that enhances their quality of life.

Location: Kingaroy, QLD

Employment Type: Full-Time

Our ideal candidate will be responsible for coordinating our clients and their assessed needs and goal plans. They will ensure that care needs are being met through high-quality service delivery.

Their tasks will include supporting clients in developing or reviewing their Client Goal/Care Plan, coordinating and monitoring the delivery of services, maintaining appropriate communication with carers and family members, and mentoring home care workers in the community.

About You:

  • Relevant experience in Home and Community Care, Case Management and/or Aged Care
  • A passion for promoting independence and self-determination by older people
  • Excellent interpersonal communication skills
  • Experience with case management and rostering software
  • Maintain a positive federal police check and Australia driver's licence
