About the Role

The Team Lead supports the Care and Support team members to provide high-quality care to our residents at all times. This involves understanding each resident's identity, culture, diversity, beliefs, and life experiences.

The Team Lead works closely with the Clinical Care Coordinator and Registered Nurse to supervise and coordinate care across the home, ensuring that individual needs, wishes, and goals are met in a supportive environment.

Your Key Responsibilities
  • Actively involved in providing resident care, knowing individual needs to ensure care plan requirements are met.
  • Providing social and emotional support to all residents, ensuring holistic and person-centered care provision.
  • Supporting the coordination of care needs across the home, including medication management, nutrition, and hydration.
  • Identifying and escalating concerns related to residents to the Registered Nurse or Clinical Care Coordinator.
  • Assisting with meal delivery and food service coordination, responding to resident requests regarding hydration and nutrition.

About You

We are seeking an individual with a Certificate IV in Ageing Support, Diploma of Enrolled Nursing (APRHA-registered) or Diploma of Enrolled Nursing (non-endorsed) with HLTHPS007 Administer & Monitor Medicines.

Applicants should possess high levels of commitment, integrity, honesty, and trustworthiness, as well as effective listening and communication skills.

Knowledge of WH&S, Infection Control, and proficiency in using information technology and programs are also essential.
