About the Role

We are seeking experienced Support Workers to join our team at Cocoon SDA Care, providing support to individuals with complex care needs in our Supported Disability Accommodation in Ballarat, VIC.

As a Support Worker, you will be responsible for assisting our participants with daily activities, personal care, and providing emotional support to enable them to live independently. You will work collaboratively with our team to develop and implement individualised plans that cater to each participant's unique needs and goals.

Key Responsibilities:
  • Assist participants with daily living activities, such as personal care, meal preparation, and mobility
  • Provide emotional support and companionship to enhance participants' quality of life
  • Develop and maintain positive relationships with participants, families, and other stakeholders
  • Contribute to the development and implementation of individualised plans that promote independence and empowerment
Requirements:
  • Cert III or higher in Disability Services or 12 months experience
  • NDIS Worker Screening Check
  • NDIS Orientation module (NDIS Quality Safety & You)
  • Working with Children Check
  • Manual handling Certification
  • Administration of Medication Certification
  • Driver's Licence
  • Car Insurance
  • Reliable vehicle

The ideal candidate will have experience working with individuals with complex care needs, manual handling skills, and a passion for supporting people to live independently. You will also possess excellent communication and interpersonal skills, with the ability to work collaboratively as part of a close-knit team.
