Job Description

The Care Navigator position at Medicare Mental Health Centre is responsible for providing high-quality, evidence-based assessment, care coordination, information, support, and treatment to individuals experiencing psychological distress. This role involves collaborating with consumers to connect them with services and supports in the community, facilitating person-centred and integrated care.

Key Responsibilities:
  • Provide practical support through information sharing and education to identify and address barriers to access.
  • Facilitate warm referrals to ensure consumers are linked to the right service at the right time.
  • Work collaboratively with the multidisciplinary mental health team to deliver holistic and collaborative care.
Required Skills and Qualifications

The ideal candidate will possess a tertiary qualification in a relevant discipline, or extensive experience in mental health, health, community services, or social sciences. Demonstrated understanding of the health and wellbeing needs of consumers with mental health and/or substance use problems is essential. The ability to engage with consumers and their families or carers to provide care navigation and referral support is also required.
