Job Overview
The Long-Term Placement Specialist will collaborate closely with team leaders to support children, young people, their siblings, foster carers, birth families, and other key individuals in achieving stable, positive, and long-lasting placement outcomes.
Key Responsibilities
- Provide trauma-informed case management guided by evidence-based practices to ensure the best possible outcomes for all parties involved.
- Support children, young people, carers, and families through practical assistance and detailed reporting requirements to build strong relationships and trust.
- Manage complex cases and coordinate a comprehensive support network to address every aspect of a child or young person's life.
- Develop and implement alternative permanency pathways such as adoption, guardianship, or long-term foster care, prioritizing the needs and well-being of the child or young person.
- Maintain professional boundaries and attention to detail in record-keeping to ensure accurate and up-to-date information.
Requirements
- Diploma or Degree in Community Services, Social Work, or a related field (or equivalent relevant experience) to provide a solid foundation for effective case management.
- A current Working with Vulnerable People Card and/or NSW Working With Children Check (as required) to demonstrate a commitment to safeguarding children and vulnerable adults.
- Full, unrestricted driver's licence to facilitate travel to meetings and visits.
- Eligibility for inclusion on the NSW Office of the Children's Guardian Residential Care Workers Register (RCWR) at the reference check stage to ensure compliance with regulatory requirements.
- A National Criminal History Check to confirm suitability for work with children and vulnerable populations.
