About the Role

We are one of the largest providers to the National Disability Insurance Scheme, supporting over 10,000 participants every year with the care they need. Our multidisciplinary team approach enables us to build a quality support service around the needs and goals of our participants and their families.

Our mission is to enhance access, development, independence, and overall health and wellbeing for people with disability, including mental health conditions.

Key Responsibilities
  • Conduct comprehensive assessments to understand individual needs through person-centred planning.
  • Develop and implement bespoke support plans that cater to each participant's unique goals and passions.
  • Collaborate with family members, carers, and other stakeholders to build capacity and strengthen relationships.
  • Work closely with a multidisciplinary team to ensure seamless delivery of support services.
