About the Role
We are seeking a dedicated professional to join our Integrated Care team as a Harm Reduction Practitioner. In this consulting-focused role, you will support clients through therapeutic assessments, harm reduction education, and coordinated care planning.
The Integrated Care zone offers comprehensive wrap-around support to people affected by drug use. This includes access to services like Opiate Agonist Therapy, BBV testing, housing support, AOD counselling, and legal and health services.
This is an opportunity to make a meaningful impact, supporting people who use drugs with dignity and respect.
Key Responsibilities
- Provide therapeutic AOD assessments, brief interventions, care planning, and secondary consultations
- Offer tailored harm reduction advice and education to meet clients' unique needs and goals
- Collaborate with Integrated Nurses and Nurse Practitioners on treatment responses
- Support the coordination of services across AOD, mental health, housing, and legal supports
- Maintain accurate and timely clinical documentation
- Develop strong referral pathways and collaborative relationships across the sector
What You'll Bring
- Relevant qualifications in AOD or related field
- Experience in AOD assessment, case coordination, and working with individuals who inject drugs
- Understanding of trauma-informed care and harm reduction principles
