Job Title: Care Coordinator - Community Support Specialist

We are seeking a compassionate and driven individual to join our team as a Care Coordinator. This is an exciting opportunity to make a real difference in the lives of people who need help most.

  • Our services include comprehensive assessments, consumer-directed care plans, and case management services.

Key Responsibilities:

  • Coordinate and manage comprehensive services and supports for clients, including building partnerships with clients and their families.
  • Undertake initial assessments and onboarding for new clients, maintaining ongoing assessment and exemplary relationships.
  • Develop, implement, and monitor consumer-directed care plans in collaboration with clients.
  • Provide case management services to clients, acting as the main point of contact between clients, their carers, and external service providers.

Requirements:

  • A strong appreciation of and ability to promote and model Christian values and ethos.
  • Tertiary qualifications in a relevant field or a minimum of three years industry experience.
  • Motivation to work with older people living in the community and an understanding of consumer-directed care approaches.
  • Proven leadership experience in overseeing, coordinating, and implementing consumer-directed care services within a home care setting.
  • Excellent oral and written communication skills, strong customer service focus, and ability to negotiate and influence others.
  • Current driver's license, good driving record, and willingness to travel for work.
