
Supportive Care Coordinator

We are seeking a dedicated and experienced professional to join our Disability Services team in delivering tailored support for clients with complex needs.
This role is ideal for someone passionate about person-centred care, especially supporting clients with challenging needs. Our ideal candidate will lead behaviour support and crisis management strategies, mentor and guide staff to build a high-performing team, engage clients and families to align services with individual goals, advocate for client rights and maintain accurate documentation, and ensure compliance with NDIS standards and policies.
Key Responsibilities:- Deliver supportive services to clients with complex needs
- Develop and implement effective behaviour support and crisis management strategies
- Mentor and coach staff to enhance their skills and knowledge
- Engage with clients and their families to understand their needs and preferences
- Promote client rights and advocate on their behalf
- Maintain accurate records and reports
- Demonstrated experience in high-intensity, person-centred disability support
- Excellent communication and interpersonal skills
- Ability to mentor and coach others
- Certificate III or IV in Disability Support (or equivalent)
- Current NSW Working with Children's Check, NDIS Worker Screening, Police Check, First Aid, and Driver's Licence
