Psychosocial Support Role
The HOPE initiative aims to provide follow-up care and support for individuals who have presented with suicide or self-harm attempt, or suicidal or intentional self-harm ideation. The program provides psychosocial support for people who traditionally would not have received such intervention immediately after hospital discharge, with broader referral pathways and targeted psychosocial support.
Key Responsibilities:
- Utilize your own lived experience of caring for someone with mental health issues to provide emotional support, focus on family/carer wellbeing, and promote choice, self-determination and greater opportunities for family members and carers to be heard.
- Work as part of a multidisciplinary team and establish strong, effective working relationships with clinical, allied health, and Consumer Peer Work staff.
- Act as recovery champions within the mental health program and may be involved in other activities including training and education, improving experience of care and service evaluation.
- Provide assertive outreach services to families/Carers in their own environment.

You are an advocate for families and carers who require support during difficult times. As a committed leader and mentor, you are dedicated to safe best practice and quality care. You are passionate about delivering patient-centred care and advocating for patients' rights.
Requirements:
- Demonstrated experience in a Family/Carer lived experience role.
- Lived experience knowledge and skills in providing peer support.
- Knowledge of peer support principles and their application in a mental health setting.
- Experience working in a collaborative team environment.
