Case Manager

We are seeking a highly skilled and experienced Case Manager to join our Pre and Post Release Support Service (PPRSS) team at Mission Australia. As a Case Manager, you will play a pivotal role in supporting individuals reintegrating into the community after incarceration, helping them achieve personal goals, secure stable accommodation, and rebuild relationships.
Key Responsibilities- Develop and Implement Reintegration Plans
Work with participants to develop and implement culturally appropriate Reintegration Care Plans, tailored to their individual needs and goals.
Collaborate with Team MembersCollaborate with team members to complete necessary risk assessments, provide participants with brief therapeutic interventions, and facilitate referrals to appropriate services.
Lead Case ConferencesLead case conferences with key stakeholders to minimize service duplication and ensure optimal outcomes for participants.
Deliver Outreach ServicesDeliver outreach services to participants, assisting them in transitioning to independence or other support services.
Foster Strong RelationshipsFoster strong internal relationships with participants and staff, contributing to the service's effectiveness and improved outcomes.
Build and Maintain RelationshipsBuild and maintain strong relationships with key external stakeholders, including referral agencies and partnership organizations.
Requirements for Success- Extensive Experience
Extensive experience in the justice or community services sector, working with individuals who have complex needs.
Demonstrated ExpertiseDemonstrated expertise in a person-centred, recovery-focused, and strength-based approach to participant care, with a deep understanding of prison release processes and community and family reintegration.
Strong Communication SkillsHighly developed verbal and written communication skills, with the ability to navigate diverse inter-organizational relationships.
Time ManagementProven ability to manage workload and time effectively, including the ability to set and achieve priorities in a high-demand work environment.
Technical SkillsAdvanced keyboard and computer skills, with competence in Client Information Management Systems.
Cultural CompetenceExperience working with Aboriginal and Torres Strait Islander people in a community service setting, along with a strong knowledge of Aboriginal and Torres Strait Islander culture.
Valid Driver's LicenseValid NT driver's license.
