About the Role

We are seeking a highly skilled and compassionate Case Manager to join our Pre and Post Release Support Service (PPRSS) team in Alice Springs. As a key member of our team, you will play a pivotal role in supporting individuals reintegrating into the community after incarceration, helping them achieve personal goals, secure stable accommodation, and rebuild relationships.

As a Case Manager, you will work closely with participants, internal and external stakeholders to deliver culturally appropriate Reintegration Care Plans, conduct risk assessments, provide therapeutic interventions, and convene case conferences. Your strong communication and interpersonal skills will enable you to build trust with participants and foster strong relationships with team members and external partners.

Key Responsibilities
  • Develop, implement, and regularly review Reintegration Care Plans tailored to each participant's needs.
  • Collaborate with team members to complete risk assessments and provide participants with brief therapeutic interventions.
  • Lead case conferences with key stakeholders to minimize service duplication and ensure optimal outcomes.
  • Deliver outreach services to participants, assisting them in transitioning to independence or other support services.
  • Foster strong internal relationships with participants and staff, contributing to the service's effectiveness and improved outcomes.
  • Build and maintain strong relationships with key external stakeholders, including referral agencies and partnership organizations.
Requirements for Success
  • Extensive experience in the justice or community services sector, working with individuals who have complex needs.
  • Demonstrated expertise in a person-centred, recovery-focused, and strength-based approach to participant care.
  • Highly developed verbal and written communication skills, with experience working autonomously and as part of a multidisciplinary team.
  • Proven ability to manage workload and time effectively, including setting and achieving priorities in a high-demand work environment.
  • Advanced keyboard and computer skills, with competence in Client Information Management Systems.
  • Experience working with Aboriginal and Torres Strait Islander people in a community service setting, along with a strong knowledge of Aboriginal and Torres Strait Islander culture.
