Job Description

This role supports individuals with disabilities in achieving their lifestyle goals, leading to a diverse and innovative work environment.

Key Responsibilities:

  • Support clients and staff to achieve their individual goals and objectives through effective planning and coordination.
  • Advocate for clients and their families to promote inclusion, participation, and respect for rights.
  • Administer medication treatments and therapies according to prescribed protocols.
  • Develop and maintain team morale and effectiveness by providing day-to-day mentoring and best practice knowledge.
  • Evaluate staff awareness of each client's support goals and ensure agreed plans are consistently implemented, monitored, and reviewed.
  • Contribute to service improvements and contribute to positive changes.
  • Update client care plans as required.
  • Communicate effectively and adapt to various situations.
  • Liaise with client family/guardian to ensure quality service delivery.
  • Prepare reports as necessary.
  • Review client records for accuracy and provide training as needed.
  • Provide direct care assistance to each client in accordance with their care plan.
