Support Coordinator - In Home Care Service
We are seeking an experienced Support Coordinator to join our team and provide exceptional support to our clients in their homes.
About the RoleThe successful candidate will be responsible for managing client files, assisting with customer management, and collaborating with health service teams to deliver high-quality care services.
- Manage client files and documentation
- Assist case coordinators with customer management and support
- Collaborate with health service teams to meet client needs
- Maintain effective communication and collaboration with clients, families, and service personnel
- Conduct comprehensive assessments of clients with their families or carers
- Determine strategies to promote independence and maintain health and wellbeing
- Coordinate safe delivery of quality holistic care and monitor client progress
- Engage with allied health professionals and community supports to enhance client care delivery
- Develop associated documents and reporting as required
- Relevant experience or background in healthcare, social work, and community services
- Basic knowledge of home-based care and NDIS services
- Proficiency in Cantonese and Mandarin, and fluency in English
- Valid Australian driver's license
- Excellent written and verbal communication skills
- Strong problem-solving skills
- Proficiency in Microsoft Office software and ability to learn computer software
