We are seeking a highly skilled and compassionate Support Coordinator to join our team. This is an exciting opportunity for a dedicated professional who is passionate about making a meaningful impact in the lives of individuals with diverse needs.

The ideal candidate will possess in-depth knowledge of the NDIS Legislative framework and Support Coordination processes. They will be responsible for conducting initial assessments to identify client needs, goals, and support requirements within the NDIS framework.

This involves developing and implementing individualized support plans in collaboration with clients and their families. The successful candidate will facilitate access to appropriate services, supports, and funding under the NDIS, provide ongoing support and coordination for clients, and advocate on behalf of clients to ensure their rights and preferences are respected.

Key responsibilities include monitoring and reviewing client progress regularly, adjusting support plans as necessary, and working collaboratively with other professionals to achieve optimal outcomes.
