The Support Coordinator involves connecting participants with a NDIS plan to supports and services in the community. The main purpose is to support and assist within Australia's NDIS participant's abilities to connect to and coordinate informal, community and funded supports in the service delivery environment.

**Main Roles and Responsibilities**
- Work with participants to find providers, services and support to achieve their goals as per their NDIS plan
- Resolve points of crisis and develop resilience with all stakeholders
- Involve participant and family in the implementation of individual packages where appropriate
- Actively participate in Professional Development
- Deliver services and support with a high level of quality and satisfaction
- Attend planning and review meetings as requested
- Keep accurate and complete records of your work activities in accordance with legislation and operational requirements
- Liaise and report to the NDIA and associated organisations
- Ensure accuracy of participant service agreement, quotes and expenditures in line with NDIS Pricing Arrangements
- Monitor spending against Participant NDIA Plan, liaise with service providers, NDIA and Plan Manager
- Ensure support coordination billable hours are align with the service agreement throughout the duration of the plan.
- Ensure Work Health and Safety guidelines are adhered to and all industry relevant legislation and codes of conduct are complied with
- Provide NDIA reports as per NDIA requirements (such as implementation report and Quarterly reports)
- Maintain an up-to-date knowledge of the NDIS systems and practices
- Performs other duties as assigned, including any non-billable activities within the organisation.
- Undertake work in line with the organisations policies, procedures, and practices
- Maintain accurate and up-to-date case notes on participant progress.

**Qualifications and Knowledge**
- Tertiary qualifications in a relevant field, such as Social Work, Service Coordination, Case Management, Allied Health and/or Disability-specific Qualifications at associate diploma level
- Current Australian Drivers Licence
- Current NDIS Worker Check
- Current Working with Children Check
- At least 2 years' experiences within the disability sector
- Understanding of NDIS Legislation and Code of Conduct
- Understanding of NDIS Restrictive Practices Guidelines
- Work Health and Safety Act 2012 (SA) (the WHS Act)
- Work Health and Safety Regulations 2012 (SA) (the WHS Regulations) and supported Codes of Practice

- Demonstrate high level interpersonal, communication and teamwork skills
- Well-developed oral and written communication skills
- Demonstrate problem solving and negotiation skills
- Ability to manage time effectively and work under pressure
- Demonstrate understanding of the changing and diverse needs of NDIS participants
- Ability to work with a high level of independence and autonomy
- An understanding of cultural sensitivity
- Proven experience in the use of Microsoft Office including Outlook, Word and Excel
- Ability to process and prioritise a high volume of activities in a fast-paced environment
- Demonstrated ability to use a planned, structured approach to allocation of resources
