The primary role of the Palliative Care Specialist is to provide clinical leadership and guidance, undertaking related projects, research and development activities specific to palliative care.
- Ensure high-quality palliative care services are provided to clients, carers/families in various settings of their choice.
- Provide expert clinical guidance to the multidisciplinary team.
- Develop and implement palliative care programs and services.
- Conduct comprehensive needs assessments and develop individualized care plans for clients.
- Collaborate with healthcare professionals to ensure seamless care transitions.
- We serve a population of approximately 100,000 across northeast Victoria.
- The region offers easy access to Melbourne, Albury airport, and numerous national parks, small towns, award-winning restaurants, wineries, and breweries.
- The area boasts diverse outdoor activities, including hiking, mountain biking, skiing, and water sports.
