About the Role

A challenging and rewarding opportunity for passionate Dementia professionals to join the Dementia Support Australia (DSA) team at HammondCare on a Fixed Term Full-Time Contract until June 2026 with possibility to extend, based out of our Chermside Office.

The Dementia Support Coach will facilitate on the ground support, implementation of relationship-based support strategies, modelling and coaching with clients, carers and care staff.

Key Responsibilities
  • Work collaboratively with peers to develop the skills, expertise and/or applied knowledge of aged care staff and carers to understand the person living with dementia and develop best practice support and care.
  • Support the implementation of the recommendation plans on the ground, drawing on modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.
  • Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.
Requirements
  • Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment and competing demands.
  • The ability to work both independently and as part of a team.
  • Demonstrated commitment to excellence in customer service principles including conflict resolution to ensure the consistent provision of exceptional service to internal and external customers
  • A current driver's licence and safe driving record
  • Willingness to travel and work intrastate and interstate for up to five days at a time to support the delivery of DC services.
  • Willingness to work flexible hours at times and as required to meet the assessed needs of clients and carers.
  • Qualification in relevant field; Allied Health assistant, Diversional Therapy, Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar highly desirable
