Job Title: Care Professional

About this opportunity:

We seek a skilled and compassionate care professional to join our team at our residential facility. As a key member of our staff, you will be responsible for providing high-quality care and support to our residents.

Your role will involve developing individualised care plans, conducting assessments, and collaborating with medical professionals to ensure the best possible outcomes for our residents. You will also be working closely with our clinical leadership team to develop and implement effective care strategies.

In addition to your clinical skills, we are looking for someone who is able to communicate effectively with residents, families, and other stakeholders. You will be an empathetic and caring individual who is able to build strong relationships with our residents and their families.

Key Responsibilities:
  • Develop individualised resident care plans and conduct regular assessments.
  • Provide high-quality nursing care and support to residents, including wound management, medication management, and palliative care.
  • Liaise with medical professionals, residents, and their families to ensure seamless care coordination.
  • Work collaboratively with our clinical leadership team to develop and implement effective care strategies.
  • Supervise and support personal care workers to ensure they have the necessary skills and knowledge to provide excellent care.
Requirements:
  • Current registration with the Australian Health Practitioner Regulation Agency (AHPRA).
  • Caring and empathetic attitude.
  • Excellent communication skills, with the ability to work effectively with diverse stakeholders.
  • Strong leadership skills, with the ability to motivate and inspire a team.
  • Good assessment and prioritisation skills, with the ability to manage competing demands.
  • Able to work effectively within a team environment.
  • Proficient verbal and written communication skills.
  • Understanding of Aged Care Quality Standards.
  • Current Police Check and influenza vaccination.
