Advanced Social Work Services Specialist

We are seeking a highly skilled and motivated Social Worker to join our team. As a key member of our Allied Health Team, you will be responsible for providing advanced social work services to patients and their families/significant others within The Alfred Hospital.
About the RoleThe successful candidate will have excellent communication and interpersonal skills, with the ability to liaise and collaborate with health professionals, family members and community agencies. They will conduct comprehensive psychosocial assessments and formulate timely quality discharge plans.
This is an excellent opportunity for new graduate social workers to develop their skills in a dynamic and supportive environment. We are looking for a professional who is able to work effectively in a multi-disciplinary team and with families, exhibiting initiative and facilitative qualities.
Key Responsibilities- Liaising and collaborating with health professionals, family members and community agencies
- Conducting comprehensive psychosocial assessments and formulating timely quality discharge plans
- Maintaining and managing a complex caseload within the clinical unit and the social work department
To be successful in this role, you will need:
- Eligibility for membership of the Australian Association of Social Workers
- Bachelor of Social Work, or recognised equivalent
- Experience in an acute hospital setting (preferable)
