Role Description:
Job OverviewAlfred Health is seeking a skilled Social Worker to join its team. The successful candidate will provide advanced social work services to patients and their families/significant others within the hospital setting.
Key ResponsibilitiesThe Social Worker role includes:
- Liaising with health professionals, family members, and community agencies.
- Conducting comprehensive psychosocial assessments to formulate and facilitate timely quality discharge plans.
- Consulting with colleagues and staff from other disciplines on issues relevant to patient care.
The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively in a multi-disciplinary team environment.
RequirementsTo be eligible for this position, you will need:
- Eligibility for membership of the Australian Association of Social Workers.
- A Bachelor of Social Work or recognised equivalent.
- Experience in an acute hospital setting is preferable.
The successful candidate will also require knowledge of working within an Electronic Medical Record system.
