We are seeking a skilled Care Coordinator to join our team at Medibank Private Limited, supporting our Community Care and My Home Hospital programs. As a key member of our Home Health team, you will play a vital role in delivering high-quality patient care and services in the comfort of our clients' homes.
Key Responsibilities
As a Care Coordinator, your primary responsibilities will include:
- Conducting comprehensive client assessments, care planning, and goal setting to ensure optimal patient outcomes.
- Coordinating in-home services to support the rehabilitation and recovery of clients, including referrals to long-term services.
- Building and maintaining effective relationships with healthcare professionals, specialists, and other stakeholders to ensure seamless care coordination.
- Monitoring patient progress, updating care plans as necessary, and providing telephone support, education, and advice to clients and their families.
- Liaising with primary and secondary care organizations to ensure continuity of care.
- Discharge planning, including referrals to long-term services and ensuring a smooth transition for clients.
About You
We are looking for a highly skilled and compassionate individual with a strong background in care coordination. As a successful candidate, you will possess:
- Minimum 2 years' experience as a Registered Nurse with unrestricted AHPRA registration.
- Recent experience in care coordination, with a strong understanding of patient-centered care.
- Excellent clinical reasoning skills, with the ability to work in a fast-paced environment.
- Strong customer service orientation, with the ability to build and maintain effective relationships with clients, families, and healthcare professionals.
- Proficient computer skills, with the ability to accurately document patient information in electronic medical records.
