About this role

We are seeking a passionate and experienced healthcare professional to provide support to our clients.

To succeed in this position, you must hold:

  • A formal tertiary qualification in nursing, such as a Bachelor of Nursing or equivalent
  • Current registration as a Registered Nurse (mandatory)
  • A national criminal history/police check
  • A valid First Aid and CPR certificate
  • A valid Blue Card

Desirable experience includes:

  • Knowledge of the aged care industry, including Community Home Care, Continuous Quality Improvement, the Aged Care Quality Principles, and the Aged Care Quality Standards
  • Previous experience in Aged Care or a related discipline
  • Ease with new software platforms

Your key responsibilities will include:

  • Building strong relationships with clients and their families
  • Conducting comprehensive client assessments and developing care plans to support clients to remain at home
  • Providing professional health and clinical advice and direction to a team of caregivers
  • Liaising with clients and their families on a regular basis, addressing concerns and service satisfaction
  • Ensuring all practices are consistent with policy and legislative frameworks
  • Maintaining up-to-date client records and information
