About the Role

We are seeking a dedicated and compassionate health professional to fill the position of Support Services Coordinator. This role is an integral part of our community team, providing support to both workers and clients.

To be successful in this role, candidates must possess:

  • A formal tertiary qualification in Nursing or equivalent field
  • Current registration as a Registered Nurse with AHPRA
  • A national criminal history/police check
  • A valid First Aid and CPR certificate
  • A valid Blue Card

The primary responsibilities of this role include:

  • Establishing strong connections with clients and their family members
  • Conducting comprehensive client assessments and developing personalized care plans
  • Providing expert health and clinical care advice to our home care workers
  • Maintaining regular communication with clients and their families
  • Ensuring up-to-date client records and information
