Care Manager

At Calvary Home Care, we are seeking a skilled Care Manager to deliver person-centered case management services to our clients. As a Care Manager, you will be responsible for coordinating care plans, working with internal and external stakeholders, and ensuring the highest level of care for our clients.
Key ResponsibilitiesDevelop and implement individualized care plans that meet the unique needs of each client.
Collaborate with healthcare professionals, family members, and caregivers to ensure seamless care coordination.
Monitor and respond to changes in client needs, adapting care plans as necessary.
Utilize innovative problem-solving skills to address complex care challenges.
Demonstrated experience in case management services, preferably in the aged care sector.
Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
Highly developed proficiency in using computers, including Microsoft Office and databases.
Understanding of financial principles and experience in monitoring and reviewing individualized budgets.
Ability to work in a team environment and negotiate with internal and external stakeholders.
NDIS clearance and police clearance (or willingness to obtain).
