
Primary Care Specialist

Key Responsibilities:- Conduct comprehensive assessments and develop effective care plans in collaboration with consumers, families, and other healthcare professionals.
- Provide timely and accurate triage, including immediate interventions and smooth transitions between acute and community services.
- Advocate for consumer rights and interests, and collaborate with service providers to ensure seamless care coordination.
- Current registration as a Registered Nurse (Division 1) with the Australian Health Practitioner Regulation Authority (AHPRA).
- Proven experience working with consumers presenting with acute mental health issues.
- Advanced reasoning skills with ability to work autonomously and as part of a multidisciplinary team with minimal direct supervision.
- Strong written and verbal communication, interpersonal skills, and ability to exercise independent professional judgement.
- Working knowledge of NSW Mental Health Act 2007 and its implications for practice.
- Ability to engage with consumers and carers regarding mental healthcare and support their recovery goals.
- Understanding of trauma-informed care principles and recovery philosophy in Mental Health.
- Current Driver's Licence and willingness to travel for work purposes.
