Job Summary

We are seeking a skilled Clinical Case Coordinator to support the Child and Youth Assessment & Therapeutic Service (CYATS) team. As a key member of our multidisciplinary team, you will play a vital role in ensuring children and young people with neurodevelopmental vulnerability receive seamless assessment, treatment, management, and support access to the National Disability Insurance Scheme.

Key Responsibilities
  • Collect and analyze background information about clients to inform case management decisions.
  • Liaise with internal and external stakeholders to facilitate effective service delivery.
  • Manage administrative tasks and provide clinical case management support to vulnerable families.
Requirements
  • Diploma-qualified or relevant experience in a clinical case coordination role or similar.
  • Demonstrated experience and skills in providing clinical case management support that is child-focused, family-centered, and trauma-informed.
  • Knowledge of child development, including neurodevelopmental disorders such as FASD, ASD, and Global Development Disorders.
