Job Description

The role of the Clinical Support Practitioner is to provide clinical knowledge and expertise to support children and young people who are residing in Out of Home Care.

The Clinical Support Practitioner works with residential care workers, family, kin and case managers to ensure children and young people who have experienced trauma receive positive support and therapeutic care.

This involves developing therapeutically appropriate plans and services, including assessments and behavior support plans. A background in clinical delivery and therapy will be an advantage.

The Clinical Support Practitioner forms part of a Multi Disciplinary outreach team providing responsive and tailored programs, assessment and therapeutic supports to children and young people across the region.

Key Responsibilities:

  • Administration and support of educational and school tasks, including one-on-one tutoring in the residential space, based on the educational needs of the children and young people in placement.
  • Providing one-on-one cultural support, including culturally specific observations, recommendations, mentoring and linking with area-defined cultural supports.
  • Transport to and from appointments (medical, clinical etc.) to have effective support from a clinical perspective.
  • Willingsness to work alongside the existing ITTC clinical team and embrace guidance, professional development and supports.
  • Completion of a cultural recommendation report, including life story work, genogram, and cultural recommendations.
  • Assistance with developing independent living skills.
  • Placement supports.
  • Assistance with risk management.
  • Engaging young people aged 12-18 years who are at risk of having a placement breakdown.
  • Creating a safe and supportive environment for young people within the ITTC program.
