About the RoleThis is an exciting opportunity to lead an effective and high-functioning team of Care Coordinators and their staff to deliver Home Care services to our valued clients.
As a Home Care Team Leader, you will be responsible for ensuring that service delivery targets are achieved, funding body requirements are met, and care and case management to Salvos Home Care consumers is coordinated and delivered as required.
Key Responsibilities- Lead and manage a team of Care Coordinators and their staff to deliver Home Care services
- Ensure service delivery targets are achieved and funding body requirements are met
- Coordinate and deliver care and case management to Salvos Home Care consumers
- Develop and maintain effective professional internal and external stakeholder relationships
- Manage a human services business and achieve best practice outcomes
- Tertiary degree-level qualification in aged care, allied health, nursing or related field, with a minimum of two years' professional work experience
- Minimum three years' work experience within Aged Care, preferably including Home Care experience, with proven ability to deliver and coordinate client-directed services within applicable compliance frameworks
- Demonstrated effectiveness in leading and managing a team of staff in an aged care, health or community services setting to achieve best practice outcomes
- Strong interpersonal communication skills and ability to develop and maintain effective professional internal and external stakeholder relationships
- Demonstrated high-level written communication skills and computer literacy
- Demonstrated understanding of and ability to support and model TSA mission, values and ethos
