Job OpportunityWe are seeking a highly skilled and experienced leader to join our team as a Home Care Team Leader. This role will lead an effective and high-functioning team of Care Coordinators and their staff to deliver Home Care services to our valued clients.
Key Responsibilities- Ensure service delivery targets are achieved and funding body requirements are met
- Coordinate and deliver care and case management to Salvos Home Care consumers as required
- Lead and manage a team of staff in an aged care, health or community services setting to achieve best practice outcomes
- Tertiary degree-level qualification in aged care, allied health, nursing or related field, with a minimum of two years' professional work experience
- Minimum three years' work experience within Aged Care, preferably including Home Care experience, with proven ability to deliver and coordinate client-directed services within applicable compliance frameworks
- Demonstrated effectiveness in leading and managing a team of staff in an aged care, health or community services setting to achieve best practice outcomes
