Job Summary

Key Responsibilities
  • Provide high-quality patient care that meets the needs of individuals with complex mental health conditions.
  • Work collaboratively with multidisciplinary teams to develop and implement patient care plans.
  • Conduct comprehensive assessments and develop individualized care plans that address the physical, emotional, and social needs of patients.
  • Lead and initiate activities to improve service delivery and patient clinical outcomes.
  • Develop and maintain effective relationships with patients, families, and healthcare providers to ensure seamless care coordination.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote best practice in community mental health care.
Requirements
  • Current registration with the Nursing and Midwifery Board of Australia with relevant post-registration qualifications and at least 3 years experience working in the clinical area of their specified post-graduate qualification.
  • Demonstrated ability to work independently and collaboratively with others, and as part of a multidisciplinary team.
  • Demonstrated knowledge and ability to mentor and coach nursing staff to enhance professional development.
  • Demonstrated ability in leading and initiating activities to improve service delivery and patient clinical outcomes.
  • Demonstrated effective clinical communication through the use of information technology and written, verbal and interpersonal skills.
  • Current NSW driver's licence and ability and willingness to travel for work purposes.
