Job Description

The Housing Case Manager is responsible for delivering high-quality interactions with clients, maintaining accurate records, and facilitating client-led assessments and care plans. This role plays a vital part in providing supports and interventions to clients, aligned with the organisation's values and vision.

This position requires a strong ability to maintain high-quality case management practices and records in line with operational requirements. The successful candidate will be able to provide targeted interventions and family group meetings to achieve client goals.
