Complex Care Specialist

The Disability Support Specialist plays a crucial role in supporting children with complex disabilities who require additional mainstream supports to sustain their care in the family home or outside it.
Key Responsibilities:- Collaborate with service providers and families to deliver high-quality support services to children with complex disabilities.
- Implement case management plans to ensure the best outcomes for children requiring alternative care placements.
- Work closely with stakeholders, including child protection and family services, to provide advice on disability and NDIS navigation.
- Engage directly with families of children with disabilities, providing them with the necessary information and support to make informed decisions about their child's care.
This role involves:
- Providing planning and coordination for children unable to live in the family home due to complex disability needs.
- Supporting transitions into adult services, ensuring a smooth process for children and young people with disabilities.
Our ideal candidate will possess excellent communication and interpersonal skills, as well as experience working with vulnerable populations. They will have a strong understanding of disability and NDIS principles, as well as the ability to work effectively in a fast-paced environment.
