The SVMH Complex Care Program provides a specialist service for consumers experiencing high levels of complexity, by working collaboratively with the consumer, treating team, services and carers.
The program provides specialist consultation, coordination and brokerage support and focuses on partnerships with relevant services (internal and external).
- Provide specialist clinical coordination by collaborating with the consumer and all services providers involved (including NDIS, SECU, forensic MH, other relevant community support services etc) to ensure they are engaged with the range of health, community and social support services required, reduce duplication of services, and establish role clarity.
- Lead and coordinate complex discharge planning from mental health bed based services (SECU, AIS, and CCU etc.).
- Engage in strategic planning and negotiation to actively keep service providers accountable, as well as engaging in collaborative problem solving.

Your Contribution

Complex Care is a consultation/coordination/brokerage service. There is very little consumer facing work, the service has a strong focus on complex discharge planning for consumers in SECU. The program also provides specialist Consultation, Brokerage support and focuses on partnerships with relevant services (internal and external).

- LOCAL WORK ENVIRONMENT
St Vincent's Mental Health is committed the recruitment and retention of high quality staff. The service aims to support staff to maintain and develop their skills to provide excellent clinical care and to meet their own personal and career goals. SVMH has a strong education and research focus in addition to affiliations with a number of universities and educational institutions. All SVMH clinical staff are expected to be familiar, and practice in a manner consistent with the National Practice Standards for the Mental Health Workforce.
- POSITION PURPOSE
Integral to the Complex Care Program is the role of Coordination. Coordination supports services to remain focussed on their roles and responsibilities in order to ensure that the established goals are being met and progress is being monitored. Complex discharge coordination for SECU consumers is also central to the role. The program also provides specialist Consultation, Brokerage support and focuses on partnerships with relevant services (internal and external).
This program is under the guidance of the Manager Footbridge Community Care Units, PARC and Complex Care, who also is the SECU Liaison Officer.
- POSITION DUTIES
- Work within the Strengths Model of Recovery orientated practice. Actively uphold a culture of hope by using optimistic language, supporting people and celebrating consumers' recovery efforts.
- Provide specialist clinical coordination by collaborating with the consumer and all services providers involved (including NDIS, SECU, forensic MH, other relevant community support services etc) to ensure they are engaged with the range of health, community and social support services required, reduce duplication of services, and establish role clarity.
- Lead and coordinate complex discharge planning from mental health bed based services (SECU, AIS, and CCU etc.).
- Engage in strategic planning and negotiation to actively keep service providers accountable, as well as engaging in collaborative problem solving.
- Attend and participate in discipline specific meetings and display a commitment to attending discipline specific training.
- Display a high standard of clinical practice and professional conduct at all times.
- Work collaboratively within the multi-disciplinary team.
- Participate in quality improvement activities; including the collection of data to inform service improvement and to monitor clinical and recovery outcomes for consumers.
- Maintenance of all documentation in line with SVMH policies, including clinical documentation, outcome measures and CMI reporting.

- Experience in the provision of assessment, discipline specific intervention and case management services for people with serious mental illness and complex needs and their carers.
- Ability to develop sophisticated care plans that are clinically informed, creative and flexible, place the consumer at the centre of their care, and which draw in services from across a range of sectors.
- An understanding of service systems, including (but not limited to) housing and homelessness, NDIS, forensic, AOD etc.
- Demonstrated leadership and problem solving skills to facilitate collaborative inter-discipline and interagency cross sector care teams which work efficiently and effectively.
- Demonstrated ability in working as part of the multi-disciplinary clinical team in promoting a standard of excellence in clinical practice.
- A comprehensive understanding and working knowledge of the Mental Health Act 2014 (including amendments) and other relevant legislation.

- Experience in the development and application of evidence based therapeutic interventions for people with major mental health disorders.
- Comprehensive understanding of the theories of human behaviour and psychopathology and their application in clinical practice.
- Knowledge of appropriate medication and administration management including relevant legislation

- Knowledge of and commitment to providing a comprehensive range of social work assessment and intervention services for people with serious mental illness and complex needs

- Post-graduate qualifications in relevant area of specialty or in the process of achieving the same are desirable.
- Experience in working within or closely with the NDIS sector is desirable.

SVHM has grouped individuals according to their risk of transmitting vaccine preventable diseases and their risk of exposure to blood or body substances (Table 2).

This includes employees with direct physical contact with patients/clients, deceased persons, blood, body substances or infectious material or surfaces/equipment that might contain these or contact that would allow acquisition and/or

This includes laboratory workers.
Category B
Vaccination is recommended for this category of HCW. This includes individuals who do not work with the risk of exposure to blood or body substances, their normal work location is not in a clinical area (e.g. chef, administrative staff) and only attends the clinical area for short periods of time. Essentially, these individuals have no greater level of risk than that of the general community.

I understand that regardless of the frequency, if I am working and or visiting in a designated 'high risk area' of SVHM (as defined in the SVHA Pre-employment/Appointment Safety Checks Policy) I will be subject to periodic Police Checks every three years at my own cost.
NDIS Clearance (if applicable):
If you are working in a designated 'Risk Assessed Role' (as defined by the National Disability Insurance Scheme NDIS) regardless of frequency, you will be subject to periodic NDIS Worker Screening Checks every five years at your own cost. 'Risk Assessed Roles' are defined as (a) key personnel as defined in the National Disability Insurance Scheme Act 2013; (b) any role that directly delivers a set of specified supports or services in the NDIS (Practice Standards – Worker Screening) Rules 2018; (c) any role where normal duties are likely to require 'more than incidental contact' with people with disability. The designation of 'Risk Assessed Roles' are subject to change, please refer to NDIS Practice Standards for further information.
Required Immunisations:
I understand that it is a condition of my employment to provide SVHM with the SVHM Staff Immunisation Questionnaire (including providing evidence i.e. vaccination cards and serological evidence) in compliance with SVHM Employee Health Screening and Immunisation Policy PRIOR TO COMMENCING WORK.
Individuals who will be working in Category A positions will only be able to commence employment following assessment of their vaccination status. The decision to proceed with the commencement of employment will be at the discretion of the ICP in consultation with the Hiring Manager and may in some instances, require additional vaccinations to ensure full compliance with the SVHM Employee Health Screening and Immunisation Policy.
