Specialized Case Manager

The organization is seeking a qualified Key Worker to provide specialist case management services to young people who are at risk and residing in family-based settings. The position will work in partnership with Child Protection and other support services, supporting children, young people, parents, and carers to deliver a holistic, trauma-informed service.
Key Responsibilities:
- Deliver intensive case management services to young people who are considered to be at risk, focusing on their individual and family issues such as abuse, family breakdown, and family relationships.
- Provide supervision and guidance to young people who require additional support.
- Contribute to the development, implementation, monitoring, and evaluation of client case plans.
- Collaborate with relevant agencies to ensure a comprehensive approach to supporting young people and their families.
Required Skills and Qualifications:
A tertiary qualification in Social Work and/or related behavioural sciences at degree level or diploma level with substantial experience is essential. Excellent written and oral communication, negotiation, and liaison skills are also required.
