Clinical Home Care Professional

This is a unique opportunity to work as a Clinical Home Care Advisor in Hervey Bay. As a key member of our care team, you will provide high-quality support to individuals in their homes, promoting independence and well-being.

Key Responsibilities:
  • Undertake assessments of participants in their homes to identify their needs and goals.
  • Develop and implement personalized care plans to ensure the best possible outcomes for each individual.
  • Monitor and communicate with participants, families, and healthcare professionals to ensure continuity of care.
  • Provide administrative support, including data entry, processing invoices, and maintaining accurate records.

You will work closely with a dynamic team of healthcare professionals and be supported by comprehensive training programs and resources.

Requirements:
  • Advanced computer skills and experience with CRM systems.
  • Strong administration skills, including prioritizing tasks and managing multiple projects.
  • Ability to work autonomously and as part of a team.
  • Current AHPRA registration as a Registered or Enrolled Nurse.
  • Minimum Bachelor's degree in Nursing or equivalent qualification.
