About the Role:

The Home Care Advisor plays a vital part in providing exceptional care and support to individuals in their homes. As a clinical or non-clinical care partner, you will work closely with clients who require assistance with daily living tasks, medical care, and emotional support.

Your Key Responsibilities Will Include:

  • Conducting thorough assessments of participants in their homes to understand their needs and preferences.
  • Developing and implementing personalized care plans that cater to the client's physical, emotional, and social needs.
  • Ongoing case management and support to clients and their families, ensuring seamless care delivery.
  • Monitoring and reporting on client progress, making adjustments to care plans as needed to ensure optimal outcomes.
  • Accurate documentation of all interactions with clients and their families, including care plans, progress notes, and other relevant information.
  • Collaboration with other healthcare professionals, such as doctors and therapists, to provide comprehensive care.
  • Participation in staff meetings and training sessions to stay updated on best practices and new developments in home care.

Requirements:

  • Advanced computer skills and experience with CRM software.
  • Strong administrative and organizational skills, with attention to detail and ability to multitask.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Excellent communication and interpersonal skills, with the ability to work effectively with clients, families, and colleagues.
  • Current CPR/First Aid certification and satisfactory police check and working with children check.
  • Minimum Bachelor's degree in Nursing or Enrolled Nurse registration, with current AHPRA registration.
