Job Title

As a Home Care Worker, you will play a vital role in enhancing the quality of life for our clients. This position involves providing personalized support to assist with daily activities, medication management, and promoting independence.

About the Role:

We are seeking an experienced and caring individual who is committed to upholding our Positive Wellbeing Model of Care. Your reliability and adaptability will be crucial in maintaining regular shifts and providing coverage during both planned and unexpected absences.

Key Responsibilities:

  • Providing personal care (e.g. showering, dressing, toileting)
  • Assisting clients with medication
  • Supporting domestic tasks and meal preparation
  • Offering social support, including transport and outings
  • Monitoring client wellbeing and updating care plans
  • Mentoring and training Home Care Workers
  • Upholding the Positive Wellbeing Model of Care

What You'll Bring:

  • Certificate III in Individual Support (Ageing)
  • Completion of key units: HLTHPS006 - Assist client with medication, HLTAAP001 – Recognise healthy body systems, CHCCCS031 – Provide individualised support.
  • Minimum 2 years' experience in a direct care role
  • Up-to-date and current First Aid and CPR certificates
  • Comfortable with technology, including proficiency with mobile apps and care software.
  • Current driver's licence and comprehensively insured vehicle
  • Strong communication and interpersonal skills, with the ability to read, write and communicate effectively in English
  • A commitment to quality, safety, and continuous improvement
