Home Care Specialist

  • We are seeking a dedicated and skilled Home Care Specialist to join our team.
Key Responsibilities:
  • Conduct assessments of participants in their homes to identify their needs and develop personalized care plans.
  • Collaborate with healthcare professionals, community support staff, and other stakeholders to ensure seamless care delivery.
  • Ensure timely follow-ups with participants to reduce risks, refer allied health services, order necessary equipment, and document progress.
  • Deliver case management services that cater to the clinical and holistic needs of participants.
  • Monitor and report on participant's wellbeing, documenting goals, progress, and outcomes using evidence-based strategies.
  • Undertake administrative tasks, including data entry, processing invoices, and ensuring compliance with regulatory requirements.
  • Maintain accurate records of interactions, communications, and evidence gathered during home visits.
  • Manage participant budgets effectively, working within government funding guidelines.
  • Work collaboratively with medical practitioners, allied health professionals, and community support staff to provide high-quality care.
  • Provide training, coaching, and guidance to community support professionals, enrolled nurses, and support staff as needed.
  • Conduct skills assessments of community support professionals performing complex care services.
  • Assist in service scheduling, planning, confirmation, and short notice placements or cancellations.
  • Engage with regulatory bodies, such as Aged Care Assessment Teams (ACAT), My Aged Care (MAC) Portal, and PRODA.
Requirements:
  • Advanced computer skills, including proficiency in CRM systems.
  • Excellent administration and organizational skills, with a focus on accuracy and attention to detail.
  • Ability to multitask and work effectively in a fast-paced environment.
  • Strong prioritization and problem-solving skills, with a results-driven approach.
  • Experience working in a wellness model, reablement, and enablement frameworks.
  • Demonstrated strengths in teamwork and autonomous work.
  • Previous experience with KPIs and meeting deadlines.
  • CPR/First Aid certification, current and satisfactory Criminal Record Check, NDIS Screening Card, and Working With Children Check.
  • Australian Driver's Licence, and previous experience in home care packages, community care, or health/aged care sectors.
  • Understanding of Home Care Package program Guidelines and Compliance, or previous community nursing experience.
  • Minimum Bachelor's degree in Nursing, with current AHPRA registration.
Preferred Qualifications:
  • Enrolled Nurse - Current AHPRA registration.
