Job Opportunity:

Supporting Individuals with Complex Needs

We are seeking experienced professionals to work in a Supported Independent Living (SIL) environment and support participants with high complex needs and medical requirements.

About the Role:

This is a dynamic opportunity for individuals who have a passion for working with people with disabilities. The successful candidate will work collaboratively as part of a dedicated team to provide tailored support services that promote independence, choice, and community participation.

Key Responsibilities:

  • Work collaboratively with participants, their families, and other stakeholders to develop and implement individualised support plans;
  • Provide person-centred support services that promote independence, choice, and community participation;
  • Assist participants with daily living tasks, including personal care, meal preparation, and medication management;
  • Develop and maintain relationships with community service providers to ensure seamless access to resources and services;
  • Maintain accurate records and reports to meet organisational and regulatory requirements;
  • Participate in ongoing training and professional development to enhance skills and knowledge;
